The social care debate is framed entirely around cost and supply. The real question<\/span>, which is<\/span> why healthspan is shrinking while lifespan grows<\/span>,<\/span> is being ignored. Until we ask why so many people spend so long in poor health before they die, no funding fix will solve the social care crisis.<\/span><\/p>\n

The Core Argument<\/span><\/p>\n

Andy Burnham's approach to social care accepts the conventional framing: the problem is cost. But the prior question<\/span>, which is<\/span> why <\/span>do we need<\/span> so much social care<\/span>,<\/span> is never asked, and without answering it, reform will always be limited.<\/span><\/p>\n

I suggest that the answer is to be found in an analysis of healthspan, which measures years of life free of chronic ill health. <\/span>The data is stark. Men's healthspan is now just 60.7 years and women's 60.9 years,<\/span> and<\/span> both<\/span> are <\/span>declining steadily. Men spend 18 years in ill health before they die,<\/span> whilst<\/span> women spend 22 years<\/span> in that state<\/span>. Since 2014, the period of ill health has risen by 2.7 years for men and 3.5 years for women. That is the<\/span> cause of the<\/span> social care crisis, and it is getting worse.<\/span><\/p>\n

Other<\/span> causes are systemic: <\/span><\/p>\n

poverty, <\/span><\/p>\n

poor housing, <\/span><\/p>\n

job insecurity, <\/span><\/p>\n

increasing retirement ages,<\/span><\/p>\n

pension insecurity,<\/span><\/p>\n

the rise of ultra-processed food, and <\/span><\/p>\n

overmedicali<\/span>s<\/span>ation<\/span>.<\/span><\/p>\n

Until those causes are addressed, demand for social care will keep rising regardless of what any politician does with budgets.<\/span><\/p>\n

The Argument Structure<\/span><\/p>\n

Step 1 \u2014 We are measuring the wrong outcome: <\/span><\/p>\n

Society celebrates rising lifespans as a success. But lifespan and healthspan are not the same thing. Men now live to 79 on average but <\/span>only enjoy 60.7 years of healthspan<\/span>. The extra years are <\/span>associated with <\/span>chronic illness. <\/span>The data is even worse for women. <\/span>A policy framework that maximises lifespan while ignoring healthspan is not a health success; it is a social care cost generator.<\/span><\/p>\n

Step 2 \u2014 Healthspan is declining, not rising: <\/span><\/p>\n

Both men's and women's <\/span>healthspans<\/span> are falling steadily. Since 2014 alone, the period of ill health before death has grown by 2.7 years for men and 3.5 years for women. Ultra-processed foods, poverty, poor housing, stress, insecurity, and overmedicali<\/span>s<\/span>ation all contribute. These are not incidental; they are structural features of how we run the economy.<\/span><\/p>\n

Step 3 \u2014 Social care demand is a symptom, not the disease: <\/span><\/p>\n

Growing demand for social care is the predictable consequence of declining <\/span>healthspans<\/span>. No budget settlement, however generous, addresses that root cause. Every pound spent managing the consequences of poor healthspan is a pound that could instead be spent preventing the poor healthspan in the first place. Burnham is treating the symptom; the disease<\/span> is going<\/span> undiscussed.<\/span><\/p>\n

Step 4 \u2014 Healthspan is a macroeconomic issue, not just a health one: <\/span><\/p>\n

People in good health in their sixties can work longer, pay more tax, claim fewer benefits, and provide informal care<\/span>,<\/span> including childcare <\/span>provided by <\/span>grandparents in an era when formal childcare is unaffordable for many families. The economic case for maximising healthspan dwarfs the cost of any social care budget. Joining those dots is what political economy demands.<\/span><\/p>\n

No one denies that people in need today must be cared for. But the question is what happens in five, ten, <\/span>or <\/span>twenty years if nothing changes.<\/span><\/p>\n

Healthspan has been declining for more than a decade. The period of ill health has risen by 2.7 years for men and 3.5 years for women since 2014 alone. Each year of delay <\/span>in addressing the root causes adds to future demand.<\/span><\/p>\n

A budget fix that ignores those trends is not a solution; it is an increasingly expensive deferral. The urgency argument cuts both ways.<\/span><\/p>\n<\/td>\n<\/tr>\n

You can't solve social care by telling people to eat better and exercise more. That's just victim-blaming.<\/span><\/p>\n<\/td>\n

Ultra-processed food now dominates UK supermarket shelves and is disproportionately consumed by people on lower incomes because it is cheaper. The food industry designs it to be addictive. That is a corporate and regulatory failure, not a personal one.<\/span><\/p>\n

Likewise, poverty, insecure work, bad housing and <\/span>later<\/span> retirement age are policy choices that create chronic stress and ill health over decades. Those are things governments can change. The argument is precisely that we should stop blaming individuals and start fixing the systems.<\/span><\/p>\n<\/td>\n<\/tr>\n

Living longer is a medical triumph. <\/span>Surely,<\/span> we should be proud of rising lifespans rather than reframing them as a problem.<\/span><\/p>\n<\/td>\n

\n

Rising lifespans are worth celebrating only if those extra years are good ones. Men now spend 18 of their final years in chronic ill health; women spend 22.<\/span><\/p>\n

That is not a triumph. It is a prolonged and often undignified period of dependency, compounded by the fact that the NHS and social care system are struggling to cope with demand they were never designed to meet at this scale.<\/span><\/p>\n

The goal should be to live better for longer<\/span> by<\/span> maxim<\/span>ising<\/span> healthspan, not <\/span>delivering <\/span>maximum lifespan at any cost. Those are different objectives, and conflating them has led us to the crisis we are now in.<\/span><\/p>\n<\/td>\n<\/tr>\n

Andy Burnham is at least trying to do something. Critics who offer no costed alternative are not helping.<\/span><\/p>\n<\/td>\n

\n

The criticism here is not that Burnham is doing nothing. It is that he is solving the wrong problem.<\/span><\/p>\n

Working within a framing that treats social care as a standalone funding question guarantees that demand will keep rising faster than any budget can be increased. The system will remain in perpetual crisis regardless of the settlement reached.<\/span><\/p>\n

The costed alternative is prevention: <\/span><\/p>\n

tackling the ultra-processed food industry, <\/span><\/p>\n

reducing poverty and housing insecurity,<\/span><\/p>\n

redesigning retirement and pension policy to remove late-life stress, and <\/span><\/p>\n

rethinking a pharmaceutical model that<\/span> can dangerously overmedicalise the elderly<\/span>. <\/span><\/p>\n

These are not vague aspirations<\/span>.<\/span> T<\/span>hey are policy levers with quantifiable economic returns.<\/span><\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n
